Do not encode or prefill sensitive information unnecessarily.

Design & artwork

A dark code on a light plain background is the safest baseline.
No text, borders or artwork touches the required blank margin.
Keep finder patterns clear and branding restrained.
Do not stretch, crop or skew the symbol to fit a layout.

Final placement & testing

Do not approve only a zoomed-in design file.
Consider glare, folds, curvature, texture and wear.
Test where the user will actually stand or hold the item.
Compatibility matters more for public or high-volume distribution.

Maintenance

Links, menus, documents and contact details can become outdated.
Keep a clean master so future edits do not depend on screenshots.

A high completion percentage is not a guarantee that every phone will scan every copy. It means you have deliberately checked the major failure points before publishing. If one essential item does not apply, document why in the notes rather than ticking it automatically.
